    rapid angiogenesis, and contains multiple cell types, including neural and glial precursor cells. The germinal matric is most active from 8-28 weeks of gestation, and normally involutes by 34-36 weeks of gestation. Anatomically, the germinal matrix lies in the space between the thalamus and the head of the caudate - the caudothalamic groove. Germinal matrix hemorrhage occurs due to stress on the fragile germinal matrix; this is due to perinatal stresses, and involved factors include altered cerebral blood flow, increased cerebral venous pressure, hypoxic-ischemic injury, among others. Premature infants are at the highest risk. The majority of these cases occur in infants born before 32 weeks of gestation, and in infants who are less than 1500 grams. There is no gender preference. The clinical presentation is variable, including failure to thrive to alterations in consciousness, seizures, hypotonia, and sudden drops in hematocrit, among others. Ultrasound is currently the standard of care for diagnosis, although MR is more sensitive and specific. On ultrasound, there is seen increased echogenicity in the caudothalamic groove, that may extend into the ventricles and parenchyma, and may cause ventricular dilatation, dependingon the grade of the bleed. Germinal matrix hemorrhage is defined by four grades.
    Grade 1 - hemorrhage confined to the germinal matrix
    Grade 2 - hemorrhage extending into the ventricle, but no ventricular dilatation
    Grade 3 - hemorrhage in the ventricle causing acute ventricular dilatation
    Grade 4 - hemorrhage within the ventricles and extending into the brain parenchyma
    The prognosis is grim, as over 50% of survivors do develop cerebral palsy, mental retardation and or hydrocephalus. At this time, treatment is supportive only.
